Host Partner Relations Truly supportive environments for people with Alzheimer's disease provide social contact with a broad array of people, greater chance for family members to stay involved with the lives of their loved ones, and access to planned community activities. One way Hearthstone residences achieve this is to offer the Hearthstone Program in a specially designed wing or floor of an Assisted Living Community. The managers, operators and owners of these co-located Assisted Living Residences for frail elders are called Hearthstone Host Partners. We are proud to have strong Host Partner relationships with premier providers of services to the elderly in Massachusetts and New York. Massachusetts Cummings Properties One of the premier owners of office buildings throughout Massachusetts, Cummings Properties owns and operates the New Horizons Retirement Community with 140 Assisted Living Apartments, a special frail elderly care wing, several independent living cottages, and the Hearthstone Program. In addition, Cummings Properties was involved in establishing the not-for-profit New Horizons at Choate Community in Woburn, Massachusetts. New Horizons at Choate includes Assisted Living Apartments, the Wayside program for frail elders, a children's day care program and medical offices, as well as the Hearthstone program. www.cummings.com Commonwealth Communities These providers of high quality Assisted Living and Nursing Home Care throughout Massachusetts manage Heights Crossing in Brockton Massachusetts, where there is a Hearthstone Residence. www.commonwealthcommunity.com Golden Pond Community Hearthstone's earliest program is located in the Golden Pond Assisted Living Residence in Hopkinton, Massachusetts, where the Boston Marathon begins each year. Golden Pond houses 110 Assisted Living apartments, a Geriatric Psychiatric floor, and a children's day care program, in addition to the Hearthstone Program on the 3rd floor. New York The Scharf Family Enterprises A well-established caregiving family, the Scharfs manage the premier Assisted Living project in New York City, The Esplanade. With 130 apartments and a well-managed Home Care Agency on site - Senior Home Care - The Esplanade serves seniors from throughout the New York area. Hearthstone's program is located on the 7th floor of the Manhattan Esplanade. There is a second Esplanade in White Plains, New York. Also a service-rich senior housing program, The Esplanade - White Plains is home to Hearthstone on the 5th floor, adjacent to a large rooftop terrace garden.
